One thing I don’t see discussed enough is design system governance, especially in larger teams. How do you handle constant changes and tweaks? It can get messy quickly when people start unlinking elements here and there.

How to manage design systems in large design teams

For design system governance, I would establish a process to manage:
1) whether an element should be added
2) if it's a one-off
3) its compliance with the system
 
While this might slow down a designer's workflow, governance is important to no break all the good work you did to setup the system.
Designers should feel free to explore, but design critiques (or system critiques) are necessary, and a clear process for ‘making it officially part of the system’ is needed.
